In order to fully understand your circumstances and needs, a MetroCare Hawaii – PLUS care coordinator will meet with you for an assessment interview. During this assessment interview, we will gather all necessary information – such as personal and medical histories, lifestyle preferences, living situation, etc. – that will help us develop the most suitable care plan that meets your home care needs.
During this interview, our care coordinators can explain service options available to you and your family, as well as answer any inquiries or concerns that you may have about our agency and services.
